HMAA SBH Comprehensive Medical Plan
for Sole Proprietors and Independent Contractors
HMAA is committed to serving the small business community and is proud to present an exclusive benefit package for the Sole Proprietors and Independent Contractors of SBH.
Plan Highlights:
- Comprehensive medical plan with dental, vision, and rx coverage
- Exclusive to SBH members only, must be in business for 3 months
- Member coverage up to age 65
- Dependent coverage up to age 26
- Rates based on age and health status
- Covers chiropractic and acupuncture care
- 80/20 plan with $2,000 annual out of pocket max
- $15 co-pay for office visit
- Physical exams $10 co-pay
- Well baby care and immunization (through age 5) covered at 100%
- Mammograms $10 copy
- Pap Smears 100%
- Prostate Specific Antigen $10 co-pay
- Employee Assistance Program (EAP) – 6 visits, no charge
- 100% coverage for oral exams, bite wing and full mouth x-rays
- Frames or contact lenses 100% up to $105, spectacle lenses 100%
- $12 co-pay for generic drugs
